Your first visit to Moore-Berry Dentistry establishes a vital foundation for our relationship with you. During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know Dr. Moore or Dr. Berry. We encourage questions and do our best to answer them thoroughly so that you feel comfortable.

Your initial appointment will consist of a thorough cleaning (unless a finding of periodontal disease is noted) with one of our dental hygienists. You’ll then have the opportunity to discuss your concerns, followed by an examination and an explanation of findings by Dr. Moore or Dr. Berry and the hygienist, with recommendations for any necessary treatment, as well as options and costs.

Please take a moment prior to your scheduled appointment to download our patient forms. We ask that you complete the forms and bring them with you to your appointment so we may better assist you in a timely manner. If you have any problem getting these files to print properly, please call our office and we will gladly send or fax them to you. Please also plan on arriving 10 minutes prior to your appointment and bring along your dental insurance card. This will save time and allow us to help you process any claims.

An extensive medical history or complex dental issues may require further evaluation and a second appointment to thoroughly review conditions and treatment options.
